Book excerpt: This report is a systematic review and critical appraisal of the literature on the use of formal outpatient diabetes education as a therapeutic tool for self-management in the adult patients with type 2 diabetes. Formal patient diabetes education to promote self-management (PDE) at different levels of complexity and comprehensiveness has emerged from instructions by physicians, nurses, and dieticians in physicianþs offices and hospitals. Since 1990 there has been a shift from traditional approaches of PDE to focus on patient-centered perspectives, self-efficacy, self-management and empowerment issuses. Quantitatives research on the use of formal PDE as a therapeutic tool promote self-management in adults with type 2 diabetes has produced mixed results in terms of improved metabolic control and reduced risks for cardiovascular disease. The long-term diabetes control outcomes of formal PDE when used to promote self-management in adults with type 2 diabetes have yet to be established. Further investigations are needed to determine which types of programs or what components are most effective in improving the ability of adults with type 2 diabetes to self-manage their disease; or which category of patients might benefit most. Findnings from qualitative research studies helped to better understand the context in which formal PDE should be applied to be successful in promoting self-managemenet behaviors in adults with type 2 diabetes.

Diabetes has become an epidemic that continues to escalate annually. Diabetes can affect multiple organ systems and lead to serious debilitating complications. Ventura County is consumed with many diabetics. There are not any comprehensive diabetes education programs in the county. Purpose/Scope: The purpose of this evidence-based practice intervention was to evaluate the effectiveness of a Diabetes self-management education (DSME) program on patient's health care utilization, medication use, self-efficaacy for diabetes and general disease management and ability to exercise, and glucose testing. A pres-test/post-test design was utilized to survey subjects participating in the program. The participants received education in four sequential sessions delivered at consistent time intervals over an eight week period. Goal: The goal of developing this program was to empower patients to develop the knowledge and skills necessary to improve health outcomes and to provide access to a diabetes self-management education program open to the general public residing in Ventura County in hopes to improve patient's lives by reducing potential long-term diabetes complications following gaining knowledge of preventative care, improving early diabetes diagnosis, and reducing the workload of primary care practices. Objectives: the objectives of this project included: following the completion of the DSME program, the subject will gain improved scores on the Chronic Disease Self-Efficacy Scales: Exercise Regularly, Manage Disease in General, Health Care Utilizations, Glucose Testing, and self-efficacy to perform self-management behaviors comparing pre-course versus post-course scores. Plan: Phase I and II involved developing the proposal draft and submitting the document to the GPD. The proposal was submitted to the IRB and approved on 3/20/2007. The class content slides were finalized. Phase III involved recruiting subjects and advertising. The classes began April 9, 2007 and continued for four separate sessions through 5/14. Pre-test and post-test questionnaires were handed out to subjects on the first day and last day of classes. Following completion, the data was analyzed. Outcomes and Results: The pre-test and post-test mean scores were compared using paired t-test and the differences were statistically significant.

Each class lasted 1.5 hours and was developed based on the American Academy of Diabetes Educators (AADE) seven essential topics for diabetes education. The program was evaluated using a pretest/posttest design. Self-efficacy was measured using the Stanford Diabetes Self-Efficacy Scale (SDSES) and self-management was evaluated using the Diabetes Self-Management Questionnaire (DSMQ). The program resulted in an increase in both self-efficacy and self-management but did not meet the program benchmark of a 20% increase. This program supports the use of DSME guided by SCT but additional work must be done to tailor the program to the target population and to reach the identified benchmark. Using a pretest/posttest approach, participants' diabetes self-management knowledge, using the Revised Michigan Diabetes Knowledge Scale (RMDKS), and fasting blood glucose were measured before and after the educational intervention session. Not all DSME programs are the same; some are certified DSME programs while others are non-certified DSME programs. The purpose of the study is to determine if there is a difference in diabetes knowledge, self-efficacy behaviors and HgbA1c for patients newly diagnosed with Type 2 diabetes who complete a certified DSME program compared to a non-certified DSME program in an outpatient setting. The proposed study will be a quasi-experimental, pre-test and post-test design. A total of 108, with 54 participants for each group, will be recruited in San Diego County. The comparison group will complete a non-certified DSME program while the experimental group will complete a certified DSME program. Three data collection instruments will be used: (1) HgbA1c lab test, (2) demographic survey, and (3) the Personal Diabetes Questionnaire. Scoring will be comparing the means of HgbA1c for both the comparison and experimental group and examining if there is a difference in the answers to the questionnaires before and after the study. The proposed study is significant to nursing because advanced practice nurses (APNs) are at a pivotal position for providing diabetes education and referring patients to the right DSME programs. APNs can provide patients the opportunity to increase diabetes knowledge and adapt attitudes and behavior to improve self-management and prevent complications. This project was based on published evidence surrounding DSME and the positive outcomes associated with educating individuals with diabetes about self-management behaviors. Methods. Participants were recruited at a rural health care clinic to include patients with insulin and non-insulin dependent diabetes over the age of 18 years that can read and speak English proficiently. Using a pretest/posttest design, participant's knowledge of self-management behaviors were measured before and after the educational intervention session using a self-developed tool, the Diabetes Self-Management Questionnaire (DMSQ). Fasting blood glucose levels were also documented prior to the intervention and post intervention by patient report via telephone. Results. Forty-two participants were included in the project. Mean pretest scores were 60% and posttest mean scores were 80%; posttest scores illustrated a 500% increase in participant's scoring {601} 80%. Overall fasting glucose decreased by only 3 points, indicating deficient applicability of the information outside the clinic setting. Conclusions. Findings suggest DSME is effective in teaching patients in the clinic setting, but more emphasis is needed to assist patients in applying this information outside the clinic setting. A diabetes self-management education program focused on educating participants about diabetes, knowledge, and skills to make informed decisions regarding their health and self-care behaviors to problem solve to improve health and health outcomes. This project involved individual educational sessions for adults 18 and older with type 2 diabetes.. The key findings concluded that participation was higher among women of Caucasian and Hispanic descent. One hundred percent of the participants increased their knowledge level. Based on these findings, individual diabetes self-management education is an effective way to improve patient knowledge of diabetes.
