Book excerpt: The comparison of chronically high (n = 37) and low (n = 37) levels of physical activity and bone mineral density (BMD) was investigated in young (12-22 years), healthy, eumenorrheic females. Subjects were placed into physical activity groups based on current and past physical activity patterns. Subjects in the high physical activity group currently engaged in an average of 10 hours per week of physical activity, while subjects in the low physical activity group exercised an average of 55 minutes per week. It was hypothesized that, regardless of measurement site, the high physical activity group would have a greater BMD when compared to the low physical activity group. Bone mineral density was obtained in the total body, lumbar spine (L1-L4), and proximal femur by dual-energy radiography, and the mid-shaft site of the bilateral radii by single photon absorptiometry. Additional measures of physique, body composition, nutritional intake, and caloric expenditure were obtained. The high physical activity group had significantly greater BMD than the low physical activity group at all measurement sites. After adjusting for weight, the significance between the physical activity groups remained in the lumbar spine, proximal femur, and femoral neck BMD. Additionally, adjustments for height resulted in a significant difference remaining in lumbar BMD. Weight, height, lean body mass, and several circumferences and skeletal widths correlated with lumbar spine, proximal femur, and total body bone mineral. The relationship between age and bone mineral measurements was linear at all of the measurement sites. There was no significant difference in the amount of calcium consumed between the physical activity groups. The results indicate that high levels of physical activity in young healthy females are associated with a greater BMD at several clinically important measurement sites, when compared to less active females, even after adjustments for size are made.

Book excerpt: Exposures during childhood that affect bone mineral density (BMD) and bone mineral content (BMC) could have long-term effects on osteoporosis development and associated fracture risk. We evaluated associations of physical activity in late childhood, adolescence, and in young adulthood with young adult BMD and BMC using data from the Dietary Intervention Study in Children (DISC) and DISC06 Follow-up Study. Moderate/Intense physical activity (MET hours/week) was assessed by questionnaire on five occasions during childhood/adolescence and again during young adulthood. BMD and BMC were measured by dual energy x-ray absorptiometry (DEXA) scans during young adulthood. Linear mixed models were used to analyze the association of physical activity with BMD and BMC during young adulthood. Generalized linear latent affects mixed models were used to evaluate the association of the latent variable "childhood physical activity" with BMD and BMC in young adulthood. Young adult BMC and BMD increased with increasing physical activity during young adulthood after adjustment for body weight, although the test for trend was significant only for BMD. Young adult BMC increased significantly with increasing physical activity during youth. Promotion of physical activity in youth and young adulthood could potentially decrease the magnitude of osteoporosis in older women as a public health problem, but more research is needed.

Although traditionally considered to be a disease affecting the elderly, increased recognition that osteoporosis may have its foundation early in life has stimulated substantial research interest into factors which can influence skeletal development, including both genetic and lifestyle factors. This book provides a summary and integrative review of the current state of knowledge of factors that influence the development of bone health during childhood and adolescence. The focus is on the role of physical activity for optimizing bone development. Other relevant topics that are reviewed include the influence of genetics, nutrition, and hormonal factors on skeletal development. In addition, there are expert views on practical lifestyle strategies and guidelines that can be adopted to optimize bone health and prevent fractures during growth. The book provides an excellent source of recent information for exercise specialists, pediatricians, nutritionists, biomedical researchers, health promotion worker and public health professionals.

Book excerpt: This first-ever Surgeon General's Report on bone health and osteoporosis illustrates the large burden that bone disease places on our Nation and its citizens. Like other chronic diseases that disproportionately affect the elderly, the prevalence of bone disease and fractures is projected to increase markedly as the population ages. If these predictions come true, bone disease and fractures will have a tremendous negative impact on the future well-being of Americans. But as this report makes clear, they need not come true: by working together we can change the picture of aging in America. Osteoporosis, fractures, and other chronic diseases no longer should be thought of as an inevitable part of growing old. By focusing on prevention and lifestyle changes, including physical activity and nutrition, as well as early diagnosis and appropriate treatment, Americans can avoid much of the damaging impact of bone disease and other chronic diseases. This Surgeon General's Report brings together for the first time the scientific evidence related to the prevention, assessment, diagnosis, and treatment of bone disease. More importantly, it provides a framework for moving forward. The report will be another effective tool in educating Americans about how they can promote bone health throughout their lives. This first-ever Surgeon General's Report on bone health and osteoporosis provides much needed information on bone health, an often overlooked aspect of physical health. This report follows in the tradition of previous Surgeon Generals' reports by identifying the relevant scientific data, rigorously evaluating and summarizing the evidence, and determining conclusions.

Available in PDF, EPUB and Kindle. Book excerpt: This is the first book of its kind to focus solely on the female athlete triad - its origins, its recognition, and most importantly, its management. Since the symptoms themselves cover a range of medical specialties, chapters are written by experts in a number of relevant fields - sports medicine, orthopedics, endocrinology, and pediatrics - with an eye toward overall care of the young female athlete. Additionally, each chapter includes suggestions on how to educate and communicate with young athletes and their parents, as well as trainers and coaches, on how to manage the illness outside of the direct clinical setting. The female athlete triad is often seen in sports where low body weight is emphasized, such as gymnastics, figure skating, and running, though it can appear in any sport or activity. The interrelated symptoms - eating disorders, amenorrhea, and low bone mass - exist on a spectrum of severity and are serious and potentially life-threatening if not properly treated. Psychological problems, in addition to medical ones, are not uncommon. The Female Athlete Triad: A Clinical Guide discusses all of these areas for a well-rounded and in-depth approach to the phenomenon and will be a useful reference for any clinician working with female athletes across the lifespan.

Although weight-bearing exercise among previously sedentary adult females can increase bone mineral density (BMD), cross-sectional studies suggest that high-intensity exercise among amenorrheic athletes may be detrimental to bone health. The long-term effects of athletic participation on BMD should be examined. Objectives. This study was designed to: (1) Determine the prevalence of varsity athletic participation among female university graduates who participated in a mail survey; and, (2) Compare BMDs of high school varsity female athletes with women who were sedentary during the same time period, while controlling for covariates. Methods. A survey was mailed to 7291 alumnae of 2 Southern California universities in the classes of 1987-1991. The survey was returned by 1900 (26.1 %), and 1373 (72.3%) consented to further contact. Of the latter, 73 athletes (70.6% of those eligible) and 67 controls (53.1 % of those eligible), all 30-35 years of age, in a clinic evaluation, provided their health histories, anthropometric measurements, physical activity and dietary assessments, and dual energy x-ray absorptiometer measurements of the hip and spine. Results. Athletes were taller (p

Book excerpt: "Physical activity, adequate energy, calcium and vitamin D intake all influence bone mineral density (BMD) in young adult females. Unfortunately, it is common to see nutrition deficiencies in this population that may put them at risk for a variety of health issues including lower BMD. Purpose: To determine the influence of the type of sport on regional and total BMD and its association with dietary energy, vitamin D and calcium intake. Methods: Seventy-three female students from McGill University were evaluated (average age of 20.8 years old, height 167.4 cm, weight 62.3 kg, and BMI 22.4). Forty-four athletes of 4 different Varsity Teams: basketball (n=13), volleyball (n=11), figure skating (n = 13), and synchronized swimming (n=7); and 29 age-matched sedentary students took part in the investigation. Dietary intake, Vitamin D and Calcium intake were assessed using a 3-day Food Log and a FFQ (short version) then analysed with the Food ProcessorTM Software. Total Body (TB), lumbar spine (LS) (L1-L4) and femoral neck (FN) BMD were assessed by DXA scanning. A one-way ANOVA explored between-group differences. Linear Regression Analysis, Pearson Correlations and ANCOVA were used to explore the influence of energy, Vitamin D and Calcium intake on BMD. Results: A significant difference in BMD at all 3 sites was observed across sports (TB = F(4,72) = 10.2, p
