Part one is a systematic literature review, in which the literature pertaining to factors that impact on the psychological wellbeing of parents of children with type 1 diabetes is reviewed. The review begins with an overview of the research relating to parents of diabetic children, which is followed by a rationale for why this review would be an interesting addition to the literature. The paper then describes the methods used to collect the relevant studies. The review synthesises the results in a way which makes it clear which factors impact on each measure of psychological wellbeing. An overview of the findings is then given. The paper closes with limitations of the review, suggestions for future research and conclusions. Part two is a preliminary pilot study in which empathy and sibling relationships are the focus. This cross-sectional study explores empathy and the quality of the sibling relationship as perceived by diabetic children, their healthy siblings and siblings in which both children are physically healthy. Based on Furman and Burhmester's (1985) model of the determinants of the quality of sibling relationships, the study moves on to explore the predictive effect of empathy on children's perceptions of their sibling relationship. The paper presents the findings and conclusions before discussing the clinical implications and the pilot study's limitations. Part three is a set of appendixes to support the work in the first two parts. It also contains a reflective account to explore the process of developing this portfolio thesis.

Book excerpt: This study was designed to examine: how siblings of chronically ill children cope with decreased parental availability, disruption to daily routines, and worries about the ill sibling; whether siblings cope differently with each of these three situations; what type of coping strategies siblings perceive as effective; the relation between psychological adjustment and the use of problem-focused and emotion-focused strategies; and the incremental variance in siblings' psychological functioning accounted for by siblings' coping strategies after controlling for age and gender. The study also explored how coping differs as a function of secondary appraisal and whether there are gender differences in coping. The Cognitive Appraisal Model of stress and coping guided the study. 110 participants (56 girls and 54 boys), 8--12 years old, completed the Kidcope, the Children's Depression Inventory, and the Revised Children's Manifest Anxiety Scale, and parents completed the Child Behavior Checklist. Results indicate that siblings' coping strategies explain a significant proportion of the variance in externalizing (14%) and internalizing (7%) behaviour problems after siblings' age and gender are controlled for, though coping is not related to child-reported symptoms of depression and anxiety. The most frequently used coping strategies are cognitive restructuring and wishful thinking. Other commonly utilized strategies are social support, distraction, problem solving, emotional regulation, and social withdrawal. Of these, social support is perceived as the most helpful in dealing with decreased parental availability and disruptions to routines, and cognitive restructuring as the most helpful in dealing with worries. Siblings employ significantly more emotion--than problem-focused strategies in dealing with all situations, though the use of problem--or emotion-focused coping is not related to secondary appraisal. Problem-focused coping in dealing with disruptions and worries is associated with higher parental reports of externalizing problems, whereas the use of emotion-focused coping in dealing with these two situations is associated with fewer such reports. There are no gender differences with regard to the relative use of problem--or emotion-focused coping. Research and clinical implications of these findings, as well as the insights and limitations of this study, are considered in the discussion.

Book excerpt: In the present study, a grounded theory approach was employed with the purpose of developing a substantive theoretical model to identify and illustrate changes in sibling relationships after pediatric bone marrow transplantation (BMT). Further investigation of the impact of sibling relationship quality on children's adaptation following this life-threatening medical procedure was examined. Two families, each consisting of a sibling dyad that included a child who received a BMT and a well sibling that ranged in age from 10-14 and a parent, participated in the study. Siblings were interviewed individually, and in the dyad, to capture an insider's view of the sibling relationship after BMT and factors that impact child adaptation to pediatric BMT. Child observational and parent survey data provided a multiple-informant and multiple-method approach which helped to foster a more complete picture of the broader family context. Findings suggest that children's sibling relationships improved after BMT, as evidenced by their building a close relationship, establishing intimacy and gaining trust, and learning to get along and settle differences better. Positive coping strategies included identifying and utilizing supports and resources, praying/prayer, making life easier for the sibling, and redefining what is important in life. Children's individual adaptation outcomes included accepting the illness, recognizing the longevity of the relationship, reducing worry/stress, and moving beyond the illness. Analyses that were grounded in the data from the present study suggest four theoretical propositions. First, children's sibling relationships have the potential for positive change after BMT. Second, close sibling relationships can serve as a buffer, or protective factor, to the adverse conditions and extremely stressful situations encountered after BMT. Third, positive changes in children's sibling relationships after BMT can positively influence children's coping strategies which may have a direct influence on children's adaptation to BMT. The fourth, and primary hypothesis, suggests that positive changes in children's sibling relationships after BMT potentiate new coping strategies in the dyad that are more facilitative to child adaptation than managing stress on one's own. Findings add to the general knowledge on sibling relationships and shed light on the complexities of children's sibling relationships when one child is severely ill.
