There is a clear clinical need for better asthma management programs in the pediatric primary care setting (Akinbami et al., 2011; Sleath et al., 2011). The current system for asthma management in primary care offices is not improving outcomes for pediatric asthma patients in the United States or the state of Montana. Barriers to providing comprehensive evidence-based pediatric asthma care exist in the primary care setting nationwide (US DHHS, 2010; & Meyer, 2011). The creation of an evidence-based pediatric asthma toolkit for the primary care provider based on the EPR-3 Asthma Diagnosis and Management Guidelines (NIH, NHLBI, 2007) is an attempt to alleviate prevalent barriers primary care providers face in pediatric asthma care. A toolkit was assembled based on literature reviewed, interviews with experts in the field, and suggestions from primary care providers after reviewing the tool. The toolkit was found to be evidence-based, appropriate for use in the primary care setting, efficient for use in a busy primary care setting, and desired for use by primary care providers in the state based on informal interviews and reviews. It is suggested that the Pediatric Asthma Toolkit for the Primary Care Provider be tested using a formal research study for its' effectiveness in improving pediatric asthma care in the primary care setting as well as improving asthma outcomes in the state of Montana. More research needs to be done in the U.S. to evaluate the effectiveness and use of asthma toolkits by primary care providers. The toolkit will need to be updated when new research is published on asthma care as well as when new national asthma guidelines are issued. The toolkit will also need to be updated with current available pharmaceuticals and devices for asthma. This professional project is in alignment with the national Asthma Control Goals for Healthy People 2020 as well as the National Asthma Education and Prevention Program recommendations for implementation of the EPR-3 Guidelines. It is also in alignment with the Montana Asthma Control Program's goals to create partnerships throughout the state and improve asthma outcomes for Montana.

Quality improvement methodology when implemented can narrow quality gaps. Objective: The objective of the project was to create and implement a plan of action to address identified gaps in key clinical activities of asthma care among pediatric population in a private pediatric setting in Northern California Methods: The project was centered on the use of Education in Quality Improvement for Pediatric Practice (EQIPP), a program of the American Academy of the Pediatrics. Both the pediatrician and the DNP student took this course and employed its methods to improve asthma management. EQIPP supports providers in improving their practice with didactic materials that help participants develop quality improvement project and tools to evaluate the outcomes of that project. Results: Based on the asthmatic patient data analysis the quality improvement team identified that the clinic lacks compliance in the following areas of national guidelines. a) Diagnosis of asthma, b) Asthma action plan and c) Asthma control and follow up. The team then developed and implemented an improvement plan based on EQIPP. Conclusion: The quality improvement project enriched the pediatric practice management of asthma patients and similar projects could be implemented in other settings too. All rights not granted by the above license are retained by the author. Abstract: Asthma is a common chronic disease for children and is a leading cause for their hospitalization. Despite its negative health impact, a local survey has shown that children with asthma as well as their parents have a lack of understanding for asthma management (Wong, Wong, Chung & Lau, 2001). However, to help asthmatic children to better cope with their condition, it is important for health care professional such as nurses to enhance their knowledge in asthma management. Recent empirical research has shown that education promoting knowledge of asthma management can improve health outcomes in asthmatic children. Therefore, it is important to develop an evidence-based asthma education guideline for them. This dissertation aims to evaluate the empirical evidence of the education program for asthmatic children, with the age ranging from 5 to 18, and their parents. The goal of the proposed program is to reduce their re-hospitalization rate by enhancing their knowledge of asthma care. A translational nursing research was conducted and 10 studies focusing on asthma education program for children and their parents were identified from electronic databases. Critical appraisal was performed using the recognized assessment tool, named the Scottish Intercollegiate Guidelines Network (SIGN) (2008). Recommendations were developed based on the data extracted from the review. Based on the literatures reviewed, it was found that a 45-to-60 minute one-to-one educational intervention conducted by nurses using self-management plan is the most effective method for improving the health outcomes for asthmatic children. A plan of translating the empirical information extracted from the review into practice was developed and the potential of implementation was assessed. Based on the assessment, it was found that there is high transferability of the findings from the review and the proposed innovation is feasible in the selected clinical setting. In addition, the benefits generated from the proposed innovation also outweigh its cost. An evidence-based guideline was then developed based on the high and medium level of evidence. The grades of the recommendation were stated as well. A comprehensive communication plan targeting on various stakeholders was prepared. A pilot study was designed to examine the feasibility of the proposed innovation before the full-scale implementation. The outcomes of the proposed innovation include the re-hospitalization rate of asthmatic children, the patients' and nurses' level of knowledge in asthma care, the satisfactory level of patients and nurses towards the innovation, the competency of nurses in conducting the innovation, and the utilization rate and the cost of the innovation. They would be evaluated using appropriate methodologies. The proposed innovation would be considered as effective if the primary outcome, the re-hospitalization rate of asthmatic children is reduced. Barriers to effective asthma management in children include the need for caregivers to identify and manage diverse environmental triggers and promote appropriate use of preventive asthma medications. Although health care providers may introduce asthma treatments and care plans, many providers lack the time and capacity to educate caregivers about asthma in an ongoing, sustained manner. To help address these complexities of asthma care, many providers and caregivers rely on patient navigators (defined as persons who provide patients with a particular set of services and who address barriers to care) (Dohan & Schrag, 2005). Despite growing interest in their value for chronic disease management, researchers and providers know little about how or what benefits patient navigators can provide to caregivers in managing asthma in children. To explore this issue, we conducted a mixed-method evaluation involving focus groups and a survey with caregivers of children with moderate-to-severe asthma who were enrolled in the Merck Childhood Asthma Network Initiative (MCAN). Findings suggest that patient navigators may support children’s asthma management by providing individualized treatment plans and hands-on practice, improving caregivers’ understanding of environmental triggers and their mitigation, and giving clear, accessible instructions for proper medication management. Study results may help to clarify and further develop the role of patient navigators for the effective management of asthma in children.

It also disproportionately affects low-income and racial and ethnic minorities. Although a wide range of interventions have been implemented to improve asthma-related outcomes among socioeconomically disadvantaged and minority children, disparities persist. This paper, which was commissioned by the JPB Foundation and conducted by MDRC in partnership with the National Academy for State Health Policy, standardizes and compares effects from rigorous evaluations across three main types of interventions: those that focus on improving education and self-management, those that focus on addressing remediation of the conditions that trigger asthma in the home environment, and those that focus on improving health care provider practice. While prior reviews have summarized findings for each intervention type, this review systematically compares findings across a range of outcomes and a spectrum of different approaches to addressing childhood asthma, drawing from about 30 independent studies and earlier published reviews. In addition, the paper includes case studies of various local programs and state-level policy initiatives to illuminate current efforts to address childhood asthma in low-income communities and barriers faced in sustaining asthma programs. Key findings include: (1) Providing education on how to manage asthma is fundamental to improving outcomes, but it may not be enough to alleviate disparities in asthma management; (2) Some local programs are clearly bridging informational gaps among low-income families and are helping them maintain a safe home environment for their children with asthma; (3) The health care benefits of interventions that go beyond education are unclear; (4) Barriers to medication management are complex and medication adherence is difficult to influence; and (5) The role of the state, and the Medicaid program in particular, is relevant for the financing and sustainability of asthma programs for low-income children. Although the long-run sustainability of some of the programs highlighted in this report is uncertain, what is clear is that combating the disparities in the prevalence of asthma among different populations will require more than an agreement on standard asthma management practices within the medical community. High rates of hospitalization, emergency room visits (ER) and infrequent prescribing of controller medications indicates the lack or inadequacies of asthma management in the home as well as primary care programs. This study examines the impact of adequate asthma management and preventive care on asthma ER visits. Methods. Data from the 2003 National Health Interview Survey (NHIS) was utilized. Children (N=653) under the age of 18 years and who have had asthma episode in the past year were included for this analysis. A composite variable was created to determine adequacy of care at home and in the primary care setting using standards from the Global Initiative for Asthma (GINA) supported by the National Heart, Lung and Blood Institute. Results. The prevalence of ER visit among children who had asthma in the past 12 months was 22.7%. Approximately a quarter (24%) of the children had inadequate asthma management and preventive care. The multiple logistic regressions analysis showed that asthma management and preventive care were important determinants for asthma ER visits. Compared to children who had adequate care, children with inadequate asthma management and preventive care were two times more likely to visit the ER for asthma related problems [OR=2.06 (95%CI=1.257, 3.361)]. Child's age, maternal education, and family income were also statistically significant determinants for asthma related ER visits. Conclusions. Practices that support early interventions for asthma management and preventive care were associated with reduced risk of ER visits. Initiatives to support mandatory asthma management education and programming at primary care practices and as well as the home setting are essential in the management of pediatric asthma.

The purpose of this initiative is to address current gaps in national childhood asthma care through clinical and nonclinical approaches to improve the management of childhood asthma. It is the first national initiative that simultaneously addresses treatment, policy, and financing issues for children with asthma at the patient, provider, and institutional levels.
